Custom forms will allow admin to create miscellaneous registration questions to be answered by players during registration check out. Custom forms can be utilized in the Programming Module and Tournaments Module. Forms can be used for single or multiple programs/categories.

Note: You can not delete a custom form, you can only edit it and remove the event that it is linked too.
Creating a New Form
Step 1: From the Profile Module, click the Custom Forms tab on the top right.
Step 2: Click Add Form below the list of current forms.
Step 3: Chose whether you are creating a form: Programming or Tournaments
Step 4: Enter a form name
Step 5: For Programming only, select all programs/categories the form should be used for upon player registration. You can also search for a program by typing the name in the search field above the scroll.
Note: You will not be able to add multiple forms on to a particular program. You will not be able to select a program if a form is already in use for that category. You can edit current forms by following the steps below.
Step 6: Click Save
Step 7: Once the form is created, click Edit field to the right of the form to create custom questions.
Step 7: Enter a field name
Step 8: Select Field type:
- Text- allow respondent to type an answer
- Radio- allow respondents to select a single choice from a list
- Check box- allow your respondents to select multiple answers from a list
- Select
Step 9: Tick ON to make questions mandatory for players to answer
Step 10: Click Add Option to make the list of choices. Options can be deleted by clicking Delete next to that option.
Step 11: For Tournaments only, you will now need to enable the Custom Forms within your tournament Settings tab. Here is a guide to follow: Custom Forms for Tournaments
Editing a Custom Form:
You will not be able to add another custom form to a program that already has a form created for it. Instead, you can edit the current form or fields.
Step 1: From the Profile Module, click the Custom Forms tab on the right.
Step 2: Click Edit to edit programs it is used for or to change the form name. Click Edit Field to edit question and choices.
Step 3: Click Save
Note: Unfortunately we currently do not have the ability to delete custom forms, you can only remove the programs or camps that they are linked to. This is on our roadmap for the future.
Viewing the Custom Form Answers for Tournaments
You will be able to download custom form answers for tournament from within your tournament, Settings tab.
Step 1: Go to the Setting tab within your tournament
Step 2: Choose the form to download and click Download data
Step 3: Await the download to complete and click the download at the top center of your screen
Viewing the Custom Form Answers for Programming
Once the person/parent completes the registration and enters the details in the custom form, you can download the custom form responses.
- Open the Programming Module,
- select the program or camp you wish to download.

Select all players and then click on the download button.
The custom form response will be the last column in the download.
Editing the Answers of Custom Forms after Submission- Programming
Once you have the custom forms, it may be that someone reaches out to say they entered the information wrong, the player/participant is not able to change this information themselves. However, you can do so in admin. Note: If the custom form was added AFTER the person registered - and they did not complete the form, you will not be able to add or edit the answers. However, you can edit the CSV download with the answers.
To Edit the Answers:
Step 1: Open the program module, select the program and then the player which you want to edit the form.
Step 2: Click on Edit Answers - this will open the pop up and you can overright what they have written. Make sure you click confirm. This will automatically update.
FAQ:
Q: I am having reports that players cannot register for the tournament and are receiving an error when brought to Additional Info area in the registration process.
A: This is usually related to a problem within the custom form you have attached. Please check that the custom form you have input in correct and no field are blank.
