Custom forms will allow admin to create miscellaneous registration questions to be answered by players during registration check out. Custom forms can be utilized in the Programming Module and Tournaments Module. Forms can be used for single or multiple programs/categories.
Creating a New Form
- From the Profile Module, click the Custom Forms tab on the top right.
- Click Add Form below the list of current forms.
- Chose whether you are creating a form: Programming or Tournaments
- Enter a form name
- For Programming only, select all programs/categories the form should be used for upon player registration. You can also search for a program by typing the name in the search field above the scroll.
Note: You will not be able to add multiple forms on to a particular program. You will not be able to select a program if a form is already in use for that category. You can edit current forms by following the steps below.
- Click Save
- Once the form is created, click Edit field to the right of the form to create custom questions.
Enter a field name
Select Field type:
Text- allow respondent to type an answer
- Radio- allow respondents to select a single choice from a list
- Check box- allow your respondents to select multiple answers from a list
- Select
Tick ON to make questions mandatory for players to answer
Click Add Option to make the list of choices. Options can be deleted by clicking Delete next to that option.
