If you need to withdraw from a league, please contact USTA Customer Care with the following information:
- Your personal information
- USTA Number
- Name
- Phone Number
- Email Address
- The name of your Flex League
- The location of where the Flex League is played (Club/Facility name, and address)
Please be aware that withdrawing will result in forfeiting any un-played matches, and these forfeited matches will not count towards your World Tennis Number (WTN).
